The state consistently ranks near the bottom nationally for cancer outcomes, and the reasons are not mysterious — they are the predictable result of systemic underinvestment in the communities that need the most support.

The Pee Dee region — the northeastern counties including Florence, Marion, Dillon, Marlboro, and Chesterfield — has some of the highest cancer mortality rates in the state, driven by a combination of deep poverty, limited healthcare access, high tobacco use, and late-stage diagnoses. This region is predominantly rural and Black, and the disparities in cancer outcomes between Black and white residents of these counties reflect decades of unequal access to screening, prevention, and treatment. Many patients in the Pee Dee drive 60 to 90 minutes to reach the closest comprehensive cancer program.

The Lowcountry — Beaufort, Colleton, Hampton, and Allendale counties — combines scenic coastal geography with some of the state’s most severe healthcare access gaps. Allendale County is one of the poorest counties in the United States, and its healthcare infrastructure is correspondingly minimal. Gullah Geechee communities along the Sea Islands and coastal areas have distinct cultural identities and, in many cases, complex relationships with the formal healthcare system rooted in historical distrust. Charleston and Columbia anchor the state’s cancer care network. MUSC Hollings Cancer Center in Charleston is South Carolina’s only NCI-designated cancer center, and Prisma Health Cancer Institute in Columbia serves the Midlands region.
